Case Manager II – Iowa Family Works (full time)

Heartland Family Service is hiring a full-time Case Manager II for our Council Bluffs office, serving families across Pottawattamie County. If you're drawn to trauma-informed work and have experience supporting families through case management, this is an opportunity to make a tangible difference.

About the Role

As a full-time Case Manager II with Heartland Family Service, you'll provide comprehensive case management services to families enrolled in Iowa Family Works. You'll work within a trauma-informed culture where your skills in assessment, advocacy, and connection-building help families access resources, build resilience, and achieve their goals in Council Bluffs and surrounding communities.

What You'll Do

• Conduct comprehensive assessments and develop individualized case plans aligned with client strengths and objectives
• Deliver direct counseling, support, and advocacy to families experiencing transitions or crises
• Identify and connect clients with community resources, services, and benefits
• Maintain ongoing contact with clients to monitor progress and adjust plans as needed
• Document all services and case activities according to program standards and regulations
• Participate in team collaboration and contribute to continuous program improvement
• Practice trauma-informed approaches that recognize client resilience and historical experiences

What We're Looking For

• Bachelor's degree in social work, counseling, psychology, human services, or related field
• Prior case management experience with families or vulnerable populations
• Excellent communication and interpersonal skills with demonstrated ability to build rapport
• Knowledge of and genuine commitment to trauma-informed care principles
• Ability to manage multiple cases and priorities independently and collaboratively
• Familiarity with Iowa community resources and service systems
• Strong ethical standards and professional integrity
