Youth and Teen Mentor

Aptivorporated is hiring a Part-Time Youth and Teen Mentor to join our team in Monona, Wisconsin, where you'll make a direct impact supporting young people and their families as they navigate transition services and build independence. If you're passionate about empowering teens with disabilities through mentorship and person-centered support, we'd like to hear from you.

Position Summary

As a Youth and Teen Mentor with Aptivorporated, you'll work directly with adolescents and teens with disabilities in the Monona area, helping them develop life skills, build confidence, and access the services and support they need to thrive. This part-time role is centered on person-centered service delivery—meaning you'll tailor your approach to each young person's unique goals, strengths, and interests. You'll collaborate closely with families, social workers, and our team to ensure a coordinated path toward greater independence and community participation.

Key Responsibilities

• Build trusting relationships with youth and families, understanding their individual needs, goals, and preferences
• Provide one-on-one mentorship and guidance focused on developing life, social, and practical skills
• Support young people in accessing community resources, educational opportunities, and employment or vocational programs
• Collaborate with families, school representatives, and service providers to ensure person-centered planning and coordinated care
• Document progress, maintain accurate records, and communicate regularly with your supervisor and team
• Model positive behaviors and encourage healthy decision-making during the critical transition years

Requirements

• High school diploma, GED, or equivalent education
• Demonstrated ability to communicate effectively with teens and families from diverse backgrounds
• Commitment to trauma-informed, person-centered approaches to youth support
• Reliable transportation and ability to travel within Monona and surrounding Dane County areas
• Valid driver's license and clean driving record preferred
• Flexibility to adjust your schedule as needed for program activities
• Passion for supporting young people with disabilities and fostering their independence
