About the Role

Join our team as a dedicated physical therapist providing evidence-based rehabilitation services in patients' homes throughout the Kirkwood area. This travel assignment places you in direct patient care, delivering individualized treatment 8 hours per day across a 40-hour weekly schedule. You'll help patients regain mobility, strength, and functional independence within a home-based setting over your 13-week engagement.

Responsibilities

• Conduct comprehensive physical assessments and functional evaluations to determine rehabilitation needs and potential
• Design individualized treatment plans tailored to each patient's condition, goals, and home environment
• Deliver therapeutic exercises, manual techniques, and modalities to improve strength, flexibility, and mobility
• Track patient progression and modify interventions to optimize clinical outcomes
• Maintain accurate, detailed documentation of evaluations, sessions, and patient response to treatment
• Educate patients and family members on prescribed home exercise programs and safety strategies
• Coordinate with physicians, nurses, and care team members to ensure integrated, patient-centered care

Who We're Looking For

• Current, valid Physical Therapist license (PT or DPT) in South Carolina
• Clinical experience in home health, acute care, or outpatient rehabilitation settings
