Role Overview

This Social Worker - Grade 1 appointment with Eastern Health is a permanent full-time post based at Angliss Hospital in Upper Ferntree Gully within the Knox Area. The package incorporates five weeks of annual leave together with a monthly day off. Core Duties

• Assist patients and families by delivering social work input across diverse clinical settings at Angliss Hospital
• Conduct psychosocial assessments and contribute to multidisciplinary care planning and goal setting
• Support safe discharge arrangements and link individuals with appropriate community or health resources
• Advocate for patient dignity, rights and wellbeing throughout the hospital episode
• Work closely with medical, nursing and allied health colleagues to maintain coordinated, person-centred support
• Maintain accurate documentation and participate in team case discussions and quality improvement activities

Requirements

• Recognised tertiary qualification in social work that meets the expectations for a Grade 1 role
• Ability to provide effective social work services to patients positioned across a range of clinical areas
• Sound interpersonal, communication and problem-solving skills suited to hospital teamwork
• Willingness to engage in continuous professional learning and to function within a supportive team culture
• Understanding of the practice standards required when assisting people experiencing acute or complex health events
