The African American Planning Commission seeks a dedicated Case Manager to join its supportive housing team in Bushwick and Williamsburg, Brooklyn. This full-time position offers the opportunity to provide direct support to residents navigating housing programs and critical life services.

The Opportunity

We're building a team committed to person-centered case management within the supportive housing sector. As a Case Manager at our Bushwick and Williamsburg locations, you'll work directly with residents to assess their needs, connect them with services, and support their path toward stable housing. This role reports to the Assistant Program Director and forms the backbone of our commitment to delivering quality supportive housing services to Brooklyn's most underserved communities.

Day-to-Day Responsibilities

  • Conduct thorough intake interviews and comprehensive assessments to understand each resident's background, circumstances, and goals
  • Evaluate eligibility for housing programs and supportive services
  • Develop individualized service plans aligned with resident strengths and priorities
  • Maintain regular contact with residents to monitor progress and adjust support as circumstances evolve
  • Coordinate referrals to community resources, mental health services, and essential support systems
  • Document client interactions and maintain accurate, confidential case records
  • Partner with team members and community organizations to ensure integrated, comprehensive care

Who We're Looking For

  • Demonstrated experience working in supportive housing or community-based social services
  • Proven skills in assessment, interviewing, and intake processing
  • Strong written and verbal communication abilities
  • Compassionate, client-centered approach to service delivery
  • Ability to work independently and collaboratively within a team environment
  • Knowledge of housing programs, eligibility systems, and social service infrastructure
  • Reliable transportation to serve our Bushwick and Williamsburg locations
