Licensed Social Worker (Fee-For-Service/Home Visit)

Child Center Residential Treatment Facility is hiring a full-time Licensed Social Worker in Queens, New York City. We're looking for a dedicated clinical professional to deliver evidence-based therapeutic services to children and families in their homes, making a direct impact on the lives of young people in our community.

About the Role

This is a full-time position in which you'll provide clinical social work services through a home visit model. You'll conduct assessments, develop treatment plans, and deliver direct therapeutic interventions to children and families across Queens who are seeking support for emotional, behavioral, and family-related challenges.

What You'll Do

• Complete clinical assessments to evaluate client needs, strengths, and treatment objectives
• Design and implement individualized treatment plans with measurable goals
• Conduct individual and family therapy sessions in clients' homes
• Maintain comprehensive clinical documentation and progress notes
• Collaborate with teachers, pediatricians, case managers, and other providers involved in client care
• Track client progress and refine therapeutic approaches based on outcomes
• Connect families with community resources and support services
• Practice in accordance with clinical standards and agency policies

What We're Looking For

• Valid New York State license (LCSW, LMSW, or LSW)
• Direct experience providing clinical services to children and families
• Competency in psychosocial assessment and treatment planning
• Strong clinical communication and therapeutic presence
• Experience in community-based or home-based service delivery is a plus
• Own reliable transportation for home visits
• Dedication to trauma-informed, culturally sensitive practice
• Ability to work independently while managing a full caseload
