Home Health Liaison

Varco Hospice is hiring a full-time Home Health Liaison in Flower Mound, Denton County. In this role, you'll bridge the gap between patients, families, and our clinical care team, ensuring seamless coordination and exceptional support throughout every patient's care journey.

About the Role

As a Home Health Liaison for Varco Hospice, you'll play a central role in connecting patients and their families with the specialized home health services they need. You'll serve as a trusted point of contact, managing intake processes, coordinating care arrangements, and ensuring that each patient receives personalized attention and clear communication every step of the way.

What You'll Do

• Conduct comprehensive patient assessments and intake interviews to understand individual care needs and preferences
• Coordinate care planning and scheduling between patients, families, medical teams, and home health providers
• Serve as the primary communication channel, answering questions and providing guidance to patients and caregivers
• Manage administrative tasks including documentation, follow-up calls, and care transition coordination
• Educate patients and families about available services, benefits, and what to expect during their care experience
• Monitor patient progress and address concerns proactively to ensure optimal outcomes and satisfaction
• Maintain accurate records and contribute to quality assurance processes within the home health program

What We're Looking For

• High school diploma or equivalent; healthcare experience or certification preferred
• Strong communication and interpersonal skills with the ability to empathize with patients and families
• Organizational ability to manage multiple patient cases and priorities simultaneously
• Comfort with electronic health records and standard office technology
• Problem-solving mindset and commitment to providing exceptional patient-centered service
• Ability to work independently and as part of an interdisciplinary healthcare team
• Valid driver's license and reliable transportation for local travel in the Flower Mound and Denton County area
