The Opportunity

As a full-time member of the team, you will serve as a trusted point of contact who helps seniors overcome barriers to care, understand available supports, and take confident next steps in their health journey. The work places you at the heart of community health navigation for older adults near Minneapolis, building stronger outcomes one relationship at a time.

What You'll Do

• Conduct thorough assessments to identify the medical, social, and practical needs of senior participants.
• Educate seniors and their families about local health programs, benefits, and preventive services.
• Coordinate referrals to clinics, home-care providers, transportation options, and social agencies.
• Follow up regularly to confirm that appointments are kept and care plans remain on track.
• Maintain clear documentation of interactions and progress so that support stays consistent.
• Collaborate with nurses, social workers, and community partners to close gaps in service.
• Empower seniors with the knowledge and tools required for independent decision-making about their health.

What We're Looking For

• Prior experience supporting seniors or working in community health navigation roles.
• Solid understanding of healthcare systems and common resources available to older adults.
• Excellent interpersonal and communication abilities that foster trust with seniors and families.
• Strong organizational skills for managing caseloads and accurate record-keeping.
• Ability to problem-solve and adapt approaches to meet the unique circumstances of each individual.
• Comfort working on-site within the Minneapolis community rather than from home.
