Position Summary

The COC Pathways Home - Housing Specialist at Scc works directly with participants to identify housing barriers, match them with appropriate units, and maintain housing once secured. In Minneapolis, Minnesota, this full-time role emphasizes person-centered planning, consistent follow-up, and clear communication with landlords, service partners, and internal teams so that every participant receives practical help navigating the local housing landscape.

Day-to-Day Responsibilities

• Complete housing needs assessments and develop individualized action plans for COC Pathways Home participants
• Conduct unit searches, schedule and attend viewings, and coach clients through lease applications and move-in steps
• Build and maintain professional relationships with property owners and managers across the Minneapolis, Minnesota area
• Coordinate deposits, utilities, and rapid-rehousing supports while tracking progress toward housing retention goals
• Document all contacts, housing placements, and outcomes accurately in required systems for ongoing program review
• Participate in regular team case conferences and cross-agency meetings to align services and remove barriers
• Provide post-placement check-ins and connection to community resources that strengthen long-term housing stability

Qualifications

• Demonstrated experience in housing navigation, case management, or related human-services work
• Working knowledge of Continuum of Care principles, fair housing standards, and local Minneapolis, Minnesota rental resources
• Strong interpersonal and communication skills for engaging diverse clients, landlords, and partner agencies
• Ability to manage a caseload independently while meeting documentation and reporting expectations
• Valid driver’s license and reliable transportation for home visits and community appointments throughout the metro area
• Proficiency with basic computer systems for case notes, email, and data entry
• Preferred background in rapid rehousing, homelessness services, or subsidized housing programs
