About the Role
As a Community Clinical Specialist with Journey Mental Health Center in Fitchburg, Dane County, you will serve as a Service Facilitator and Care Coordinator within the PROPs First Episode Psychosis program. PROPs delivers care largely through Dane County's Comprehensive Community Services (CCS) framework, a collaborative model offering a wide range of mental health and substance use supports. In this role, you'll help each participant build and guide a provider team of their own choosing, ensuring services are person-centered, coordinated, and responsive to individual goals.
Key Responsibilities
- Coordinate and facilitate services for youth and young adults enrolled in the PROPs First Episode Psychosis program.
- Help participants assemble a team of providers that reflects their preferences and needs.
- Connect individuals to mental health and substance use resources available through the Dane County CCS system of care.
- Support continuity of care by aligning services across providers and the broader community.
- Advocate for participants and promote recovery-oriented, individualized treatment planning.
Qualifications
- Background in mental health, behavioral health, social services, or a closely related field.
- Familiarity with care coordination and community-based service delivery, ideally within a CCS or similar system.
- Strong communication and collaboration skills for working alongside multidisciplinary provider teams.
- Commitment to serving youth and young adults experiencing first-episode psychosis with empathy and respect.
- Ability to work on-site in Fitchburg, Dane County (this is not a remote position).
