What This Role Involves

Utilize your clinical expertise to navigate and support tangata whai ora (people in need) as they transition into the community. Your role will involve guiding them towards the necessary resources and support systems to ensure their well-being and success.

Day-to-Day Responsibilities

• Assess individual needs of tangata whai ora to determine appropriate interventions and services.
• Collaborate with healthcare providers, community organizations, and other stakeholders to facilitate access to care.
• Educate tangata whai ora on available resources and support options.
• Monitor progress and adjust plans as necessary to ensure ongoing success.

Skills & Qualifications

• Proven experience in clinical navigation or a related field.
• Demonstrated ability to work effectively with diverse populations.
• Strong communication and interpersonal skills.
• Affinity for linking people with the right resources and support systems.
