What This Role Involves

This role involves providing phone and community-based support to connect whānau (families) with local social and Hauora (well-being) support services. The position is focused on creating meaningful connections between people and the resources they need within their communities.

Key Responsibilities

• Assist whānau in accessing community resources and support services.
• Build strong, collaborative relationships with local organizations to enhance community support networks.
• Provide emotional and practical support to individuals and families as needed.
• Maintain accurate records of interactions and support provided.
