About the Role
Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our Children's Long-Term Support (CLTS) Waiver Program team in Walworth County, Twin Lakes, Wisconsin. This full-time position offers a hybrid work option and requires working Monday through Friday during the first shift.
Responsibilities
- Provide direct care coordination to children and families enrolled in the CLTS Waiver Program
- Ensure compliance with state and federal regulations for long-term support services
- Develop and maintain comprehensive care plans for each client
- Collaborate with interdisciplinary teams, including social workers, therapists, and medical professionals
- Monitor client progress and adjust care plans as needed
- Communicate effectively with families, caregivers, and other stakeholders to ensure the best possible outcomes for clients
What You Bring
- A minimum of a bachelor's degree in social work, nursing, or a related field
- Previous experience working with children and families in a care coordination role
- Strong communication and interpersonal skills
- Familiarity with long-term support services and regulations
- Ability to work collaboratively within an interdisciplinary team
