Care Coordinator – Lutheran Social Services of WI & Upper MI About the Role Lutheran Social Services of Wisconsin & Upper Michigan is seeking a dedicated Care Coordinator to join our Children's Long-Term Support (CLTS) Waiver Program team in Walworth County, Twin Lakes, Kenosha County. This full-time position offers a hybrid work option and requires working Monday through Friday during the first shift.
