Bluebird Hospice is hiring a dedicated Licensed Practical Nurse (LPN) for our Bluebird Home Health team to deliver compassionate, hands-on care to patients across the United States (nationwide). About the Role

As a Licensed Practical Nurse with Bluebird Home Health, you'll provide direct patient care under the guidance of a registered nurse. Following each patient's individualized plan of care, you'll administer treatments and partner closely with the broader care team to ensure every patient receives safe, attentive, and high-quality home health services.

Key Responsibilities

• Deliver direct, hands-on patient care in accordance with the established plan of care
• Administer prescribed treatments and monitor patient progress under registered nurse supervision
• Collaborate effectively with members of the interdisciplinary care team to coordinate patient needs
• Document care accurately and communicate any changes in patient condition to the supervising RN
• Uphold professional nursing standards while supporting patients in a home-based setting

Qualifications

• Current, valid Licensed Practical Nurse (LPN) license in good standing
• Ability to follow a patient plan of care and work under the supervision of a registered nurse
• Strong collaboration and communication skills for working within a care team
• Commitment to compassionate, patient-centered home health care
• Availability for full-time work within the United States (nationwide)
