The Opportunity
SG Enable is seeking a seasoned professional to lead and support the Future Care Planning Resource Centre (FCPRC) in shaping the future care planning landscape. As a key figure, you will drive the development of standards and tools for FCP practice, guide complex caregiving cases, and foster collaborations with government, private, and community partners.
Day-to-Day Responsibilities
- Lead the Future Care Planning (FCP) practice within FCPRC
- Develop and implement standards and tools for FCP
- Mentor practitioners in complex caregiving cases
- Foster strategic partnerships with government, private, and community organizations
- Drive capability development initiatives to enhance the skills of FCP professionals
What You Bring
- A minimum of 10 years' experience in healthcare or related fields
- Demonstrated leadership in developing and implementing care planning frameworks
- Strong interpersonal and communication skills to mentor practitioners and build partnerships
- Experience in strategic planning, project management, and change management
- A deep understanding of the future care landscape and its implications for policy and practice
