Registered Nurse (Ohio)

What This Role Involves

The Registered Nurse in this position serves as a clinical leader within an interdisciplinary care team, focusing on individuals with serious illness. You'll conduct clinical assessments and implement screening protocols to identify member priorities, develop individualized care strategies in collaboration with team members, and coordinate care through both internal and cross-team meetings. This role leverages modern clinical systems and tools to deliver integrated, technology-enabled care while building meaningful connections with members and caregivers.

Core Duties

• Conduct and interpret clinical assessments and screening instruments throughout the care journey
• Develop person-centered care plans working within the interdisciplinary team framework
• Facilitate coordination meetings and clinical rounds to optimize member outcomes
• Navigate and utilize electronic health systems and practice management tools
• Foster empathetic, trust-based relationships with members, families, and care partners
• Lead collaborative care efforts across network providers and care management teams
• Demonstrate clinical judgment in recognizing when escalation is necessary during urgent situations

Who We're Looking For

• Current, unrestricted Registered Nurse license in Ohio with no board sanctions
• Background in clinical or medical settings (preferred)
• Familiarity with interdisciplinary care models and coordinated care networks
• Strong written and verbal communication abilities
• Capacity to manage multiple priorities under pressure while maintaining service quality
• Sound technical skills across documentation systems and electronic platforms
• Valid Ohio driver's license, safe driving record, and required auto insurance
• Comfort navigating organizational change and evolving priorities
• Spanish language proficiency (preferred)
