Outreach & Navigation Specialist

Role Overview

As an Outreach & Navigation Specialist with Sonoran Prevention Works, you'll be the bridge between our community and the services and support systems they need. This hybrid position, based in Pima County, places you at the heart of our Community Programs department. You'll work as a non-supervisory team member reporting to the Outreach & Navigation Manager, delivering direct support to individuals and families seeking help and guidance. As a full-time, non-exempt role, you're eligible for overtime compensation and will be part of a mission-driven organization working to strengthen public health and community resilience.

Day-to-Day Responsibilities

• Conduct outreach activities to identify and connect with community members who can benefit from our services
• Guide individuals through the process of accessing social services, health programs, and community resources
• Provide information sessions and educational presentations to community groups and at public events
• Document interactions and maintain detailed records of client contacts and outcomes
• Collaborate with partner organizations to ensure coordinated care and comprehensive resource awareness
• Support program participants with follow-up communication and assistance navigating systems

Qualifications

• High school diploma or equivalent required; some college coursework or professional experience in community outreach, social services, or case management preferred
• Strong communication skills and ability to work effectively with diverse populations
• Reliable transportation and valid driver's license for hybrid work involving community visits in Tucson and Pima County
• Demonstrated commitment to public health, community service, or social support
• Ability to work independently while maintaining regular contact with your manager and team
• Proficiency with basic computer systems and willingness to learn database and reporting tools
