About the Role

As a Visiting Nurse and primary care RN with Hartford HealthCare at Home, you'll work in the home care setting—building relationships with patients, managing their complex medical needs, and coordinating their care plan. This is nursing where personal connection matters: you'll see the same patients over time, understand their homes and families, and adapt your care to each person's unique circumstances. Based in Simsbury with service territory spanning Hartford County, this full-time position offers the clinical autonomy and patient continuity that many nurses seek in home-based practice.

What You'll Do

• Conduct comprehensive patient assessments, vital signs monitoring, wound care, medication management, and other skilled nursing interventions in home settings
• Develop and implement individualized care plans aligned with physician orders and patient goals
• Educate patients and caregivers on disease management, medication compliance, and self-care techniques
• Document clinical findings, progress notes, and outcomes in our electronic health record system
• Communicate with physicians, interdisciplinary teams, and insurance providers to coordinate seamless care transitions
• Respond to patient changes in condition and escalate concerns appropriately

What We're Looking For

• Current Connecticut RN license (or willingness to obtain) with active CPR certification
• Experience in direct patient care, acute care, primary care, or home health nursing preferred
• Strong clinical assessment and critical thinking skills with ability to work independently
• Excellent communication and documentation abilities
• Reliable transportation and valid driver's license required
• Compassion for patients across diverse backgrounds and health conditions
• Flexibility to manage a dynamic caseload and adapt to individual patient needs
