About This Position
The PoWER Lead Community Health Worker (CHW) position at Cdcfoundation in Kentucky (statewide) (statewide) is a full-time role with an annual salary ranging from $63,000 to $80,000, depending on experience. The position will support the Kentucky (statewide) (statewide) Rural Health Transformation Program (RHTP), focusing on expanding timely prenatal and postpartum care through telehealth-enabled maternal care teams.
Core Duties
- Support the implementation of PoWERing Maternal and Infant Health program to enhance access to prenatal and postpartum care in rural areas.
- Collaborate with local communities, healthcare providers, and other stakeholders to identify and address gaps in maternal health services.
- Recruit, train, and support community health workers to deliver culturally sensitive and effective health interventions.
- Develop and maintain partnerships with various organizations to leverage resources and expand the reach of PoWERing initiatives.
Qualifications
- Minimum of a high school diploma or equivalent; a bachelor's degree in public health, nursing, social work, or related field is preferred.
- Proven experience as a Community Health Worker or similar role with a focus on maternal and infant health.
- Strong interpersonal skills to build trust and rapport with community members.
- Affinity for working in rural communities and commitment to improving healthcare access and outcomes.
