The Opportunity
The Clinical Document Specialist at Franciscan Health is pivotal in enhancing the quality and completeness of clinical documentation. This role involves extensive interaction with physicians, coding staff, case managers, and other healthcare team members to ensure accurate medical record documentation that reflects diagnoses treated, procedures performed, and patient severity of illness.
Day-to-Day Responsibilities
- Facilitate modifications to clinical documentation through concurrent interaction with physicians and coding staff using the query process to clarify information, obtain needed documentation, present opportunities, and educate for appropriate identification of severity of illness.
- Communicate effectively with physicians, case managers, coders, and other healthcare team members to ensure comprehensive medical record documentation that reflects clinical treatment, decisions, and diagnoses for inpatients.
- Utilize expertise in understanding complications, co-morbidities, severity of illness, risk of mortality, case mix, secondary diagnosis, and the impact of procedures on final DRG; share this knowledge with physicians and other members of the healthcare team.
- Identify patterns, trends, variances, and opportunities to improve documentation review and processes.
Qualifications
- Associate's degree in Nursing or a related field - Required
- Bachelor's degree in Nursing or a related field - Preferred
