Youth Housing Coordinator – YHHS

Scc is hiring a full-time Youth Housing Coordinator - YHHS in Minneapolis, MN to help young people navigate housing pathways, build stability, and access the supports they need in the Twin Cities area.

Position Summary

The Youth Housing Coordinator - YHHS role centers on guiding youth through housing assessment, placement, and retention. You will connect participants with appropriate living options, coordinate with partners across Minneapolis, MN, and provide steady case-level follow-through so youth can move toward safe, lasting housing. This full-time position calls for organized advocacy, clear documentation, and a collaborative approach with youth, families, and community resources.

Day-to-Day Responsibilities

• Meet with youth to assess housing needs, goals, barriers, and readiness for placement
• Identify suitable housing opportunities and support applications, referrals, and move-in steps
• Coordinate with landlords, housing providers, case partners, and community agencies serving Minneapolis, MN
• Develop and track individualized housing plans, including short-term crisis options and longer-term stability goals
• Monitor progress after placement, address risks of housing loss, and help youth problem-solve tenancy issues
• Maintain accurate case notes, eligibility records, and required reports in a timely manner
• Educate youth on tenant rights, lease expectations, budgeting for housing costs, and available local supports
• Participate in team meetings and coordinate warm handoffs so services stay continuous for each young person
