Position Summary
This Case Associate role is central to New Alternatives for Children, Inc.'s mission of providing specialized support to children and families with medical complexities, disabilities, chronic illnesses, and behavioral challenges. You'll work within our Specialized Foster Care Program, coordinating care, maintaining family connections, and ensuring every child receives person-centered support tailored to their unique circumstances and development.
Key Responsibilities
- Conduct thorough assessments and develop individualized case plans for children and families
- Coordinate services across medical, educational, and behavioral health providers
- Maintain regular contact with foster families, biological families, and placement resources
- Document case progress, outcomes, and service delivery in accordance with regulatory standards
- Facilitate communication and collaboration among all stakeholders in a child's care network
- Support placement stability and family reunification goals aligned with each case plan
- Ensure compliance with child welfare regulations and agency policies
Qualifications
- Bachelor's degree in social work, psychology, counseling, or related field, or equivalent professional experience in child welfare
- Demonstrated commitment to serving vulnerable children and families with cultural competence and trauma-informed practices
- Strong organizational and written communication skills
- Ability to manage multiple cases and competing priorities while maintaining meticulous documentation
- Valid driver's license and reliable transportation for in-person client visits
- Willingness to engage in ongoing professional development and training
