New Alternatives for Children, Inc. (NAC) is hiring a full-time Prevention Case Planner (Bachelor and Master Level) in Meadows, Hudson County to support children and families living with medical complexities, chronic conditions, and disabilities.

About the Role

As a Prevention Case Planner with New Alternatives for Children, Inc., you will join NAC's Special Medical Prevention Program, helping vulnerable families remain safely together. This on-site position in Meadows, Hudson County is open to candidates at both the bachelor's and master's level, offering a meaningful path to strengthen family stability and child well-being.

Key Responsibilities

• Coordinate prevention services for children and families affected by medical complexities, chronic illness, and disabilities.
• Develop, monitor, and update individualized service and safety plans for each family on your caseload.
• Conduct regular home visits and family meetings to assess needs, progress, and risk.
• Connect families with community resources, medical providers, and supportive services.
• Maintain accurate, timely case documentation in line with program and agency standards.
• Collaborate with supervisors, multidisciplinary teams, and external partners to advance positive outcomes.

Qualifications

• A bachelor's or master's degree in social work, human services, or a related field.
• Interest in or experience supporting children and families with medical and developmental needs.
• Strong assessment, organizational, and case management skills.
• Clear written and verbal communication for documentation and family engagement.
• Ability to work on-site in Meadows, Hudson County in a full-time capacity.
