Lancesoft INC is hiring an experienced RN Case Manager for our healthcare facility in Fort Walton Beach, Florida. This is a full-time day shift position offering competitive compensation with up to $10,000 in sign-on bonus and relocation assistance for qualified candidates.

About the Role

As an RN Case Manager, you'll ensure patients receive seamlessly coordinated, evidence-based care throughout their healthcare journey. Working days (8:00 AM – 4:30 PM) with rotating weekend schedules, you'll combine clinical nursing expertise with strategic care coordination to achieve positive patient outcomes and facilitate smooth transitions across care settings.

What You'll Do

• Assess and manage patient cases to develop individualized care plans aligned with clinical and social needs
• Coordinate discharge planning and arrange continuity of care to prevent readmissions
• Serve as the central liaison between patients, families, clinicians, and community support resources
• Track patient progress, evaluate care effectiveness, and adjust interventions as clinically indicated
• Partner with interdisciplinary teams—physicians, social workers, therapists, and specialists—to optimize safety and outcomes
• Connect patients with available community services, insurance resources, and long-term support options

What We're Looking For

• Current RN license in Florida or eligibility to obtain licensure
• Demonstrable clinical judgment and sound decision-making in complex patient situations
• Excellent oral and written communication skills
• Prior experience in care coordination, case management, or related healthcare roles
• Strong ability to collaborate effectively with diverse interdisciplinary teams
