About the Role

You'll serve as a trusted advocate and educator for families navigating early childhood. This is hands-on, community-based work where you'll build lasting relationships with parents and caregivers, helping them create nurturing environments where children thrive. Your role bridges the gap between families and the resources they need to support healthy child development.

What You'll Do

• Conduct regular home visits to assess family strengths, challenges, and developmental needs
• Provide guidance on child development, parenting strategies, and family wellbeing
• Connect families to community resources, health services, and educational programs
• Observe and document child progress using evidence-based screening tools
• Coordinate with health professionals, educators, and social services on behalf of families
• Respond to crisis situations with empathy and appropriate referrals
• Maintain detailed case notes and program documentation
• Collaborate with your team to ensure families receive comprehensive support
