Service Coordinator

Lutheran Social Services is hiring a Service Coordinator to support our integrated programs serving individuals and families in Pell Lake, Walworth County. This full-time position plays a vital part in connecting people to the resources and care they need, from child and family support to addiction recovery and emergency assistance.

As a Service Coordinator, you'll be a bridge between clients and the comprehensive programs Lutheran Social Services offers to our community. Your work directly impacts the lives of individuals and families seeking support during critical transitions and challenges. You'll help ensure that every person receives coordinated, compassionate care tailored to their unique circumstances.

What You'll Do

• Coordinate intake processes and conduct assessments to understand client needs and goals
• Maintain accurate, confidential client records and documentation
• Connect clients with appropriate programs, including child and family services, addiction support, and disaster response initiatives
• Schedule appointments and manage calendar logistics for clients and program staff
• Provide follow-up support and track client progress through their service journey
• Collaborate with social workers, case managers, and other team members
• Respond to client inquiries and troubleshoot barriers to accessing services
• Support program operations and contribute to a welcoming, inclusive client experience

What We're Looking For

• Strong organizational and time-management skills

• Excellent written and verbal communication skills
• Comfort with confidentiality and HIPAA-related compliance
• Proficiency with computer systems and office software
• Ability to work effectively as part of a mission-driven team
• High school diploma or equivalent; some human services experience preferred
