Service Coordinator

About the Role

As a Service Coordinator with Lutheran Social Services, you'll connect families with the support services they need to thrive. You'll assess client needs, develop personalized service plans, coordinate referrals across community partners, and track progress toward meaningful goals. Your work will directly influence how families stabilize their situations and move forward with confidence.

What You'll Do

• Conduct thorough intake assessments to understand family circumstances, strengths, and challenges
• Build and maintain comprehensive service plans that align with client goals
• Coordinate referrals to social services, healthcare, mental health, financial assistance, and housing support
• Follow up with clients and service partners to ensure continuity of care and progress monitoring
• Document all activities, outcomes, and service engagement in organizational systems
• Maintain confidentiality while building trusting relationships with vulnerable populations
• Collaborate with Lutheran Social Services staff and community agencies to address complex family needs

What We're Looking For

• High school diploma or equivalent; bachelor's degree in social work, human services, or related field preferred
• Demonstrated commitment to supporting families and promoting self-sufficiency
• Strong organizational and communication skills, both written and verbal
• Ability to manage multiple cases and competing priorities with professionalism and empathy
• Experience with case management, social services, or community-based support (or willingness to learn)
• Reliable transportation for occasional in-person client visits within the service area
• Problem-solving mindset and genuine compassion for people facing adversity
