Position Summary

We are seeking a dedicated Outreach Therapist to join our team at Community Health Resources in Manchester, Connecticut. This role focuses on providing functional family therapy for foster care adolescents within the community. The ideal candidate will have experience and passion for working with youth in a therapeutic setting.

What You'll Do

• Provide individual and group therapy sessions to foster care adolescents using Functional Family Therapy techniques.
• Collaborate with families, caregivers, social workers, and other professionals to develop comprehensive treatment plans.
• Conduct home visits to assess family dynamics and provide support in a natural setting.
• Maintain accurate records of therapy sessions and progress reports for all clients.
• Participate in team meetings and ongoing training to enhance therapeutic skills and knowledge.

What We're Looking For

• A minimum of a Master's degree in Social Work, Psychology, or related field.
• Licensed as an LCSW, LMFT, or LMSW in the state of Connecticut.
• Experience working with foster care adolescents and their families.
• Strong communication and interpersonal skills.
• A commitment to cultural competency and diversity.
