Practitioners manage prevention, illness encounters, and long-term conditions while building patient relationships and upholding standards for the organization plus its hospital network collaborators. Full-scope comprehensive practice is expected, covering pediatric patients and behavioral supports. Home-based delivery draws on AI clinical tools plus continuous backing from concierge, operations, and technology colleagues so focus stays on care quality rather than paperwork.

Core Duties

• Conduct video comprehensive clinical visits spanning acute needs for adults and children along with mental health concerns
• Maintain regular weekend coverage consisting of 7 a.m. to 7 p.m. Saturday and Sunday plus one weekday for a 36-hour weekly total
• Aid physicians through Epic inbox handling of labs, imaging, and patient inquiries
• Instruct patients on fitting treatments and health plans
• Issue prescriptions and renewals when clinically proper
• Advance strong outcomes-focused clinical performance
• Strengthen virtual experiences with genuine compassion
• Develop agile solutions for emerging issues or escalate correctly
• Partner effectively with care team colleagues
• Achieve demanding objectives within a high-caliber group

What You Bring

• Minimum five years of Family Nurse Practitioner clinical work inside primary care environments
• Active licenses in Florida, New Jersey, New York, Massachusetts and/or an NLC compact credential
• At least one year of hands-on Epic EMR experience
• Current Medicare provider enrollment status or eligibility to enroll
• Background treating primary care plus behavioral health for both acute and chronic conditions
• Comfort with technology, proactive habits, strong organization, attention to detail, and prior telemedicine work
• Unblemished background check alongside clean malpractice record
• Availability for the stated weekend and weekday 36-hour schedule
