About the Role
As a full-time Case Management Coordinator, you'll coordinate comprehensive care services within our specialized residential communities. Working closely with residential teams, service providers, and the individuals we support, you'll ensure every person receives person-centered, coordinated care that meets their unique needs and goals.
What You'll Do
- Develop and update individualized service plans based on comprehensive assessments of each person's needs, strengths, and preferences
- Coordinate communication between residential teams, healthcare providers, families, and community partners to ensure seamless service delivery
- Monitor progress toward personal and therapeutic goals, making adjustments to care plans as needed
- Advocate for the individuals you serve, ensuring their voices are heard in all care decisions
- Maintain comprehensive, accurate documentation that meets regulatory and compliance requirements
- Support crisis resolution and connect individuals with appropriate resources and referrals
- Participate in team meetings and contribute to creating a supportive, inclusive residential environment
What We're Looking For
- Experience in case management, social services, residential support, or related field
- Understanding of person-centered planning and commitment to empowering individuals with disabilities
- Strong communication and relationship-building skills across diverse teams and populations
- Ability to manage multiple priorities, maintain detailed documentation, and meet deadlines
- Proficiency with electronic records systems and Microsoft Office applications
- Bachelor's degree in social work, psychology, healthcare, or related field preferred; equivalent professional experience considered
- Valid driver's license and reliable transportation
