About This Position

The Care Manager, LTSS (Registered Nurse) role at Molina Healthcare in Detroit, Michigan involves providing support for care management and coordination activities related to long-term services and supports. As a key member of the multidisciplinary team, you will ensure integrated delivery of high-quality care across various settings for members with complex needs. This position is full-time and remote-friendly.

Key Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans, working closely with members, caregivers, physicians, other healthcare professionals, and the member support network to address specific needs and goals.
• Monitors care plan effectiveness, documents interventions, and suggests changes as needed.
• Promotes integration of services for members, including behavioral health care, long-term services and supports (LTSS), and home and community resources.
• Evaluates medical necessity and authorizes appropriate waiver services.
• Advices on covered benefits and funding sources to members.
• Facilitates interdisciplinary care team meetings for service approvals or denials and informal collaboration.
• Uses motivational interviewing techniques and Molina clinical guideposts to educate, support, and motivate change during member contacts.
• Assesses barriers to care and provides coordination and assistance to address psycho/social, financial, and medical obstacles.

What You Bring

Candidates should possess a Registered Nurse (RN) license in the state of Michigan or equivalent. Experience in long-term services and supports is preferred. Strong communication skills, ability to work independently, and proficiency with electronic health records are essential. Knowledge of motivational interviewing techniques and familiarity with Molina Healthcare’s clinical guideposts will be advantageous.
