Role Overview

This PACE RN Case Manager - Citrus County opening at Chaptershealth in Lecanto, Florida centers on guiding comprehensive, participant-focused care within an interdisciplinary setting. You will serve as the primary clinical navigator for enrollees, balancing acute and chronic needs while promoting independence and quality of life throughout Citrus County. The role demands clinical judgment, proactive planning, and seamless collaboration to keep every care plan responsive and person-centered.

Your Responsibilities

• Perform thorough nursing assessments of new and ongoing participants to identify clinical, functional, and psychosocial needs within the PACE model of care.
• Design, implement, and regularly update individualized plans of care in partnership with the full interdisciplinary team and the participant’s family or caregivers.
• Coordinate home- and center-based services, specialist referrals, medication management, and durable medical equipment so that participants in Lecanto and surrounding Citrus County areas receive timely support.
• Monitor health status community visits and telephonic check-ins, promptly addressing changes that could lead to avoidable hospitalizations or emergency visits.
• Educate participants and families on disease management, medication adherence, wellness strategies, and self-care techniques that foster aging in place.
• Document all assessments, interventions, and communications accurately in the electronic health record while ensuring alignment with regulatory and organizational standards.
• Serve as a clinical resource for team members and contribute to continuous quality-improvement activities that strengthen outcomes for the PACE population.

Who We're Looking For

• Active Registered Nurse license valid in the state of Florida and readiness to practice as a PACE RN Case Manager - Citrus County.
• Solid background in case management, community health, geriatrics, or chronic-care coordination, ideally gained through prior PACE, hospice, home-health, or population-health experience.
• Demonstrated ability to complete comprehensive assessments, prioritize competing clinical demands, and adjust care plans based on evolving participant conditions.
• Excellent interpersonal and communication skills for collaborating with physicians, therapists, social workers, dietitians, and community partners.
• Comfort conducting visits in both center and home environments across Lecanto, Florida and greater Citrus County, with reliable transportation and flexibility for a full-time schedule.
• Proficiency with electronic medical records, care-management software, and standard Microsoft Office tools for efficient documentation and reporting.
• Strong organizational habits and a commitment to culturally sensitive, person-directed nursing practice that respects the dignity of older adults.
