Who We're Looking For Active Registered Nurse license valid in the state of Florida and readiness to practice as a PACE RN Case Manager - Citrus County. Solid background in case management, community health, geriatrics, or chronic-care coordination, ideally gained through prior PACE, hospice, home-health, or population-health experience. Demonstrated ability to complete comprehensive assessments, prioritize competing clinical demands, and adjust care plans based on evolving participant conditions.
