The Opportunity

Luna Del Valle HCS is seeking a dedicated Hospice Medical Social Worker to deliver person-centered supportive services to hospice patients and their loved ones across Socorro County. In this part-time role, you'll work closely with an interdisciplinary care team to help families navigate the physical, emotional, and practical challenges of end-of-life care. Compensation is $30–$35 per visit, with benefits including health insurance and paid time off.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments and develop individualized care plans aligned with patient and family goals
  • Provide individual and family counseling, crisis intervention, and emotional support throughout the hospice journey
  • Facilitate communication between patients, families, and the interdisciplinary care team
  • Connect patients and families with community resources, financial assistance programs, and social services
  • Assist with discharge planning and transitions in care
  • Maintain accurate, timely clinical documentation and collaborate on care coordination efforts

Requirements

  • Master of Social Work (MSW) degree and current state licensure (LCSW or LMSW) in New Mexico
  • Demonstrated experience or strong interest in hospice, palliative care, or end-of-life services
  • Excellent verbal and written communication skills
  • Ability to work effectively with diverse patient populations and family systems
  • Current driver's license and reliable transportation for in-home visits
  • Commitment to ethical, culturally responsive, and patient-centered social work practice
