Case Manager

Northeast Family Services seeks a full-time Case Manager to join our team in Berlin, Coos County. As an established mental health provider with over 20 years of experience supporting our communities, we're looking for someone passionate about helping individuals and families access the care and resources they deserve.

About the Role

In this position, you'll serve as a vital bridge between clients and the mental health services that support their recovery and well-being. As a Case Manager at Northeast Family Services, you'll coordinate comprehensive care plans, connect people with appropriate treatment and community resources, and advocate for clients' needs as they work toward stability and growth.

What You'll Do

• Assess client needs, strengths, and barriers to care through comprehensive intake and evaluation processes
• Develop and maintain individualized care plans that align with client goals and treatment objectives
• Coordinate referrals and connect clients with mental health services, community resources, and support programs
• Maintain regular contact with clients, families, and treatment team members to monitor progress and adjust support as needed
• Document case activities and client progress in compliance with clinical and regulatory standards
• Advocate on behalf of clients to ensure equitable access to appropriate services and accommodations
• Participate in clinical team meetings and supervision to support positive client outcomes

What We're Looking For

• Bachelor's degree in social work, psychology, counseling, or a closely related field
• Demonstrated experience working with individuals experiencing mental health or behavioral health challenges
• Strong communication and interpersonal skills with the ability to work effectively across diverse teams
• Excellent organizational abilities and attention to detail
• Compassion and cultural competency when working with vulnerable and diverse populations
• Proficiency with case management software and electronic health records
• Ability to manage multiple cases and priorities in a fast-paced clinical environment
