About the Role
The Case Manager position at the Mather Community Campus, Singles Program plays a vital role in our mission to help individuals rebuild their lives. You'll work directly with program participants to assess needs, develop individualized support plans, and coordinate resources that facilitate their progress toward independence and dignity.
What You'll Do
- Conduct comprehensive assessments of client needs, strengths, and barriers to self-sufficiency
- Develop and implement individualized case plans aligned with client goals and preferences
- Provide ongoing support, coaching, and advocacy to program participants
- Coordinate and facilitate access to community resources, benefits, and social services
- Maintain detailed and timely documentation of client interactions and progress
- Collaborate with internal teams and external partners to ensure comprehensive support
- Track outcomes and contribute to program data and reporting
- Attend team meetings and engage in professional development activities
What We're Looking For
- Bachelor's degree in social work, counseling, human services, or related field
- Experience in case management, client services, or program support
- Strong communication and interpersonal skills
- Ability to work compassionately with individuals experiencing homelessness or housing instability
- Excellent organizational and documentation abilities
- Capacity to work independently and collaboratively within a team environment
- Genuine commitment to helping others achieve stability and independence
