Youth in Transition Outcome Specialist

Join Northeast Treatment Center as a Youth in Transition Outcome Specialist in Philadelphia, Pennsylvania. This full-time position is part of NET Community Care, where you'll directly support young people as they navigate critical life transitions and work toward sustainable independence.

About the Role

As a Youth in Transition Outcome Specialist, you'll play a vital role in helping adolescents and young adults achieve meaningful progress toward self-sufficiency. Working within the Youth Engagement and Advocacy department, you'll combine individual support, strategic planning, and collaborative coordination to remove barriers and unlock opportunities for the young people you serve. Reporting to the Director of Youth Engagement and Advocacy, you'll be part of a mission-driven team committed to evidence-based youth development in the Philadelphia area.

What You'll Do

• Build trusting relationships with young people and assess their strengths, challenges, and goals across education, employment, housing, and life skills
• Develop and execute individualized transition plans that connect youth to resources, services, and community opportunities
• Coordinate with schools, employers, social services, and other community partners to align support and remove obstacles
• Document progress, track outcomes, and maintain detailed case notes to ensure continuity and accountability
• Advocate on behalf of young people within systems and institutions, amplifying their voice and needs
• Participate in team meetings, professional development, and organizational initiatives that strengthen youth outcomes

What We're Looking For

• Genuine passion for supporting youth in transition and a commitment to equity and empowerment
• Strong communication, organizational, and problem-solving skills
• Ability to build rapport, demonstrate cultural competence, and work effectively across differences
• Experience in youth services, case management, social work, education, or related fields (preferred)
• Reliable, professional, and able to work collaboratively within a team environment
• Bachelor's degree or equivalent experience in a related field
