About the Role

This part-time position offers the opportunity to make a direct, meaningful impact on patients' lives. As a Home Health Medical Social Worker with AdvisaCare, you'll provide psychosocial assessments, care coordination, and resource navigation—helping individuals and their families successfully manage complex health and social needs while remaining safe at home. You'll work autonomously in patients' homes across Macomb County, building trusting relationships and connecting families with the community resources and support they need.

What You'll Do

• Conduct comprehensive psychosocial and environmental assessments for home health patients
• Coordinate care plans with interdisciplinary teams and community providers
• Connect patients and families with essential social services, mental health resources, and financial assistance programs
• Provide counseling and emotional support to patients and caregivers facing health crises or life transitions
• Advocate for patient needs and work to remove barriers to accessing care
• Maintain accurate, timely clinical documentation and communicate with the clinical team
• Support discharge planning and transitions between care settings when appropriate

What We're Looking For

• Master of Social Work (MSW) degree with valid Michigan LMSW or LCSW licensure
• Experience in home health, healthcare, or community-based social work
• Strong clinical assessment and problem-solving skills
• Excellent communication abilities with diverse populations
• Reliable transportation and ability to travel throughout Macomb County
• Compassion, cultural humility, and commitment to patient-centered care
