About the Role
As a Registered Nurse in Primary Care, you'll deliver direct patient care within a collaborative medical team environment. This role combines clinical assessment, patient education, and care coordination to support individuals managing acute and chronic health conditions. You'll work alongside physicians and care staff to ensure continuity of care and patient engagement with their treatment plans.
What You'll Do
- Perform patient intake assessments, including vital signs monitoring and health history review
- Execute nursing interventions and administer medications according to clinical protocols
- Educate patients about preventive care, chronic disease management, and medication adherence
- Document patient encounters and clinical findings accurately in electronic health records
- Coordinate referrals to specialists and community health resources
- Support care planning and population health initiatives
- Maintain a safe, organized clinical environment
What We're Looking For
- Active Connecticut Registered Nurse (RN) license
- Clinical nursing experience, with primary care or ambulatory care preferred
- Strong interpersonal and patient communication skills
- Ability to work independently and as part of an interdisciplinary team
- Proficiency with electronic health record systems
- Knowledge of evidence-based clinical practices and patient safety standards
- Commitment to delivering compassionate, high-quality patient care
