Social Work p.r.n. is seeking a Master of Social Work professional to serve as a Geriatric Care Manager in New York City. This is a full-time, permanent role providing comprehensive care coordination and advocacy for older adults across the city and surrounding boroughs.
About the Role
In this position, you'll deliver individualized care management to support older adults in navigating complex health and social needs. You'll work directly with clients and families, coordinating services across healthcare and community systems while advocating for their wellbeing and independence.
What You'll Do
- Conduct comprehensive assessments of clients' physical, cognitive, emotional, and social needs
- Develop individualized care plans aligned with each client's goals and circumstances
- Coordinate services across medical, social, and community providers
- Monitor client progress and adjust care plans as needs change
- Advocate for clients' rights and wellbeing within healthcare and social service systems
- Build and maintain strong relationships with clients, families, healthcare providers, and community partners
- Document care activities and maintain accurate, confidential records
- Support care transitions, including medical events, placements, and long-term planning
What We're Looking For
- Master of Social Work (MSW) degree from an accredited program
- Expertise in geriatric care, aging-related health conditions, and resources for older adults
- Strong communication and interpersonal skills with diverse populations
- Ability to manage multiple client caseloads independently and effectively
- Genuine commitment to supporting older adults and improving quality of life
- Proficiency with electronic health records and standard office software
- Valid driver's license with reliable transportation for client visits throughout the city
