Housing Care Manager – Gannett House

Hudson River Housing is seeking a full-time Housing Care Manager to join our team in Arlington, Dutchess County. In this role, you'll provide comprehensive support and advocacy services to help families and individuals achieve housing stability and reach their personal goals.

About the Role

As a Housing Care Manager at Hudson River Housing, you'll serve as a dedicated professional supporting our clients through direct case management and coordinated care. You'll work closely with families and individuals in our programs, offering practical assistance, resource connection, and ongoing support tailored to each person's unique circumstances and aspirations.

What You'll Do

• Meet with clients to understand their needs, challenges, and objectives, then develop individualized service and support plans
• Provide crisis intervention and immediate assistance during urgent situations
• Connect clients with community resources, social services, and other agencies to support their progress
• Deliver consistent guidance, encouragement, and advocacy throughout the client relationship
• Maintain thorough, accurate documentation of client interactions and outcomes
• Monitor client progress toward goals and adjust support strategies as needed
• Participate in team meetings and collaborate with colleagues and partner organizations
