About the Role

The Care Transition Unit serves patients moving through critical phases of recovery and discharge planning. As Team Lead on nights, you'll oversee unit operations during evening and overnight hours, supporting both your nursing team and the patients and families in your care. This is a full-time position offering the leadership opportunity to shape nursing practice while maintaining hands-on clinical involvement.

What You'll Do

• Lead and mentor the nursing team during night shifts, fostering professional development and ensuring high standards of patient care
• Provide direct patient care including assessment, treatment planning, and patient/family communication
• Coordinate care transitions, working with interdisciplinary teams to support safe discharge and recovery
• Monitor unit operations, manage resources, and ensure compliance with clinical protocols and safety standards
• Respond to complex patient situations and guide your team through clinical challenges
• Document care accurately and communicate shift handoff information to incoming teams

What We're Looking For

• Registered Nurse (RN) licensure in the State of Florida
• Proven experience in acute care nursing or care transitions
• Leadership experience or demonstrated readiness to lead clinical staff
• Strong assessment skills, clinical judgment, and communication abilities
• Commitment to patient-centered care and team collaboration
• Flexibility to work night shift schedules
