Role Overview

Swams is seeking an experienced and dedicated Home Stretch Coach to join our team on a fixed-term 6-month contract. The Transition Coach for Homestretch plays a vital role in supporting individuals in their journey towards independence and self-sufficiency.

What You'll Do

• Provide comprehensive support to clients transitioning from hospital or residential care settings back into the community.
• Develop and implement individualized transition plans that focus on personal growth, health, and well-being.
• Collaborate with healthcare professionals, social workers, and other stakeholders to ensure a coordinated approach to client care.
• Maintain accurate records of clients' progress and communicate regularly with team members and clients.
• Identify and address barriers to independence, working closely with clients to overcome challenges.

What You Bring

• A minimum of 3 years of experience in a similar role, preferably within the healthcare or social services sector.
• Strong interpersonal and communication skills, including the ability to build trusting relationships with clients.
• Excellent organizational and time management abilities to handle multiple client cases effectively.
• Proficiency in Microsoft Office Suite and other relevant software for record-keeping and reporting.
• A deep understanding of community resources and services available to support transition needs.
