The Transition Coach for Homestretch plays a vital role in supporting individuals in their journey towards independence and self-sufficiency. What You'll Do Provide comprehensive support to clients transitioning from hospital or residential care settings back into the community. Develop and implement individualized transition plans that focus on personal growth, health, and well-being. Collaborate with healthcare professionals, social workers, and other stakeholders to ensure a coordinated approach to client care. Maintain accurate records of clients' progress and communicate regularly with team members and clients. Identify and address barriers to independence, working closely with clients to overcome challenges. What You Bring A minimum of 3 years of experience in a similar role, preferably within the healthcare or social services sector. Strong interpersonal and communication skills, including the ability to build trusting relationships with clients. Excellent organizational and time management abilities to handle multiple client cases effectively. Proficiency in Microsoft Office Suite and other relevant software for record-keeping and reporting. A deep understanding of community resources and services available to support transition needs.
