Social Worker – Intermediate Care

Liquid Personnel is recruiting a Social Worker to support an Intermediate Care program in Southwark London Borough. This full-time, six-month fixed contract role is within an integrated health and social care service dedicated to helping residents achieve recovery and independence.

About the Role

The Intermediate Care Service spans five areas across Southwark, providing support to vulnerable residents as they progress through recovery and reablement. As a Social Worker, you will be part of a small, interdisciplinary team committed to delivering person-centered, strength-based support. You'll help address service demand while ensuring residents receive timely, high-quality interventions that promote autonomy and positive outcomes.

What You'll Do

• Conduct and review Care Act assessments and develop personalized care and support plans
• Carry out Mental Capacity Assessments and participate in Best Interests decision-making
• Identify safeguarding concerns, evaluate risk, and establish effective protection measures
• Work across boundaries with health services, community resources, and informal support networks to deliver comprehensive, individual-focused care
• Document cases accurately and contribute to organizational learning and reflective practice

What We're Looking For

You are a registered Social Worker who is passionate about empowerment and independence. You work with a strength-focused, person-centered orientation and are comfortable operating within interdisciplinary environments.

• Experience in integrated care settings, reablement services, or rehabilitation programs
• Strong knowledge of social care legislation, policy, and safeguarding frameworks
• Commitment to equality, diversity, and inclusive practice
• Strong communication, assessment, and organizational skills
• Ability to manage competing priorities while keeping residents' needs and autonomy central
