Concord Hospital is seeking a full-time Patient Care Coordinator to support our Internal Medicine practice in Concord. You'll work Monday through Friday, 8:30 AM to 5:30 PM, handling key administrative and patient-facing responsibilities under the direction of the Practice Manager.
About the Role
The Patient Care Coordinator position is central to the day-to-day operations of our Internal Medicine practice. You'll serve as the primary administrative resource for patient access, clinical support, and care coordination, ensuring smooth communication and efficient operations across the department.
Key Responsibilities
- Manage patient registration and intake processes to ensure accurate, complete records
- Coordinate referrals and facilitate communication with referring providers and specialists
- Handle phone management, directing patient calls and inquiries to appropriate resources
- Perform clerical duties including scheduling, correspondence, and file maintenance
- Execute data management functions and maintain accuracy in electronic health records and administrative databases
- Provide administrative support to the Practice Manager and clinical team
What We're Looking For
- Strong organizational and multitasking abilities in a clinical setting
- Professional written and verbal communication skills
- Comfort with computers and experience with electronic health records systems, or demonstrated ability to learn new systems
- Meticulous attention to detail and commitment to accuracy
- Understanding of patient confidentiality, HIPAA compliance, and healthcare regulations
- High school diploma or GED; medical office experience or relevant certification (MA, CMA, or CPC) preferred
