Licensed Clinical Social Worker (LCSW) – Part-Time / PRN

BrightSpring Health Services is seeking a Licensed Clinical Social Worker (LCSW) - Part-Time / PRN to join its neuro community‑based care team in Norristown.

About the Role

This position provides psychosocial support to patients and families coping with neurological conditions, delivering care in home and community settings to promote emotional well‑being and functional independence.

What You'll Do

• Conduct biopsychosocial assessments and develop individualized care plans for clients with neurological diagnoses.
• Provide therapeutic counseling, crisis intervention, and coping strategy education to patients and their caregivers.
• Coordinate with interdisciplinary teams, including physicians, therapists, and case managers, to ensure seamless service delivery.
• Document interactions accurately in electronic health records while maintaining confidentiality and compliance with regulatory standards.
• Facilitate referrals to community resources, support groups, and ancillary services that address social determinants of health.
• Participate in ongoing training and quality improvement initiatives specific to neuro‑rehabilitation populations.

What We're Looking For

• Current, active Licensed Clinical Social Worker (LCSW) credential in the state of Pennsylvania.
• Master’s degree in Social Work from an accredited program.
• Experience working with individuals affected by stroke, traumatic brain injury, multiple sclerosis, Parkinson’s disease, or similar neurological conditions.
• Strong clinical skills in assessment, psychotherapy, and case management within community‑based or home‑health environments.
• Excellent communication abilities, cultural humility, and a collaborative mindset.
• Valid driver’s license, reliable transportation, and willingness to travel within Montgomery County.
• Basic proficiency with electronic medical record systems and standard office software.
