A Community Of Friends is seeking a Case Manager I (ICMS) in Willowbrook, Los Angeles County to join our permanent supportive housing team on a full-time basis. In this role, you'll provide intensive case management services to formerly homeless individuals and families with complex medical and behavioral health needs who require coordinated support and care.
About the Role
As a Case Manager I, you'll deliver comprehensive intensive case management services (ICMS) to a vulnerable population, including individuals and families who are formerly homeless and experiencing chronic medical conditions alongside co-occurring mental health or substance use disorders. Many of your clients will be high utilizers of emergency and hospital services within the Department of Health Services system. Your case management work will focus on stabilizing housing, connecting clients to healthcare and social services, and supporting their path toward stability and independence.
What You'll Do
- Assess client needs and develop individualized case management plans with clear goals and service coordination
- Maintain ongoing contact with clients through office visits, phone support, and field work in their homes and communities
- Coordinate with healthcare providers, mental health services, and community resources to ensure comprehensive care
- Assist clients with accessing benefits, housing programs, and social services for which they may be eligible
- Document client progress, case notes, and service delivery in required systems and formats
- Provide crisis support and intervention when clients face emergencies or acute challenges
- Advocate for client needs and collaborate with interdisciplinary teams

- High school diploma or equivalent
- Strong communication and interpersonal skills with the ability to build trust with diverse, vulnerable clients
- Demonstrated experience or commitment to working with formerly homeless populations and individuals with chronic illnesses or behavioral health challenges
- Organizational skills and ability to manage multiple client cases simultaneously
- Compassion, cultural humility, and dedication to client-centered care
- Ability to maintain strict confidentiality and professional boundaries
- Valid driver's license and reliable transportation
