About the Role
As a Clinical Care Transition Educator / Registered Nurse with Option Care Health, you will guide patients and their families through the transition into home and alternate-site infusion therapy. Based in Strathmoor Manor, this part-time RN role blends clinical expertise with patient education to ensure safe, well-supported care at every step.
Key Responsibilities
- Educate patients and caregivers on home infusion therapy, equipment, and self-care techniques
- Coordinate smooth care transitions between hospital, clinic, and home settings
- Assess patient needs and reinforce treatment plans developed by the clinical team
- Monitor patient progress and document care in accordance with clinical standards
- Collaborate with physicians, pharmacists, and fellow clinicians to elevate the standard of care
Qualifications
- Active Registered Nurse (RN) license in the state of Kentucky
- Clinical nursing experience, ideally in infusion, home health, or care coordination
- Strong patient-education and communication skills
- Ability to work independently and manage transitions of care with attention to detail
- Commitment to compassionate, patient-centered service
